The gun that killed Steve fired a bullet made of radion, a substance lethal to Jack Kirby characters was actually a gizmo designed by Doom and Arnim Zola that "froze him in time and space"; another gizmo "unstuck" him, causing him to hurtle through history, leaving clues to his stops along the way relive the events of his life in random order, alá Slaughterhouse Five. The whole thing was yet another scheme by the Red Skull to take Steve's Perfect Body for his own.

the Wikipedia article is a pretty good summary. The miniseries wasn't bad, overall, but it was plagued by publishing delays that kinda spoiled things, because, by the time the final issue came out, everyone had seen Steve alive and well in SIEGE.
