What's going on with the Buffy cartoon? I'm probably missing something, but I don't quite get it.

It's an allusion to the 5th's Grey's Anatomy. Karev had a five-year-old patient who believed she was a superhero but actually only had a condition in which one is born without proper pain reception. The way they test for pain reception is they make you hold your hand in a bucket of ice.
